Re: Transcript request from St Elizabeth's Hospital S.O.N.

For those of you who have had a difficult time obtaining your nursing school transcripts from St Elizabeth's Medical Center, thank you for your patience. The changes during COVID have been challenging at best but I believe we have restructured a process that will be simple and satisfactory for all.

I have sent the new form to our web designer today mindful of their present staffing issues. When you check the Resource page on the web site the form to download will have a March 10,2022 revision date. Do not use the form with the October 2016 revision date.

In the meantime, please send your request via email to Mackenzie.robinson@steward.org along with your name at graduation, the year of your graduation, a number where you can be reached. Please allow a 5 day lead time. If you have any difficulties, please call me @ 508 317 1438
